• Original and two copies of certificate of registration/incorporation,
• Memorandum and Articles of Association
• Resolution to open the account and account mandate

Group/Church Account

• Certificate of registration and minutes certified by the Department of Social Services,  board or committee resolution
• Letter from the church or parish minister specifying the account signatories and the account operating mandate
